THIS IS NOT AN OFFICIAL LUCENT DOCUMENT
A few years ago I prepared a document that my wife could use to assist her in the event of my death. I divided this document into two parts, one dealing with Lucent benefits which she would be eligible for as a survivor, and the other, a listing of information dealing with Social Security benefits, private insurance policies, and other important documents.
That document was specific to my benefits as a medicare eligible "MANAGEMENT/NON-REPRESENTED PLAN DESIGN RETIREE" who retired prior to March 1, 1990 and was covered by the Traditional Indemnity Plan.
The current document, SURVIVOR ACTIVITIES -(UPDATED FEBRUARY 2008), is also based on the benefits of retirees whose benefits are covered under the same category. The information on health benefits may be different for other retirees due to their date of retirement, Medicare eligibility, management vs. occupational pension plans, etc. I believe that most of the information in this document is not limited to those retirees in the above category, and can be used by most of the Lucent retirees as well. Addition information may have to be obtained by you to insure that your spouse will have the proper information to obtain their benefits.
After all my effort in preparing the document for my wife, I thought that I would prepare a Generic Document that might make it a little easier for other retirees to have this same type of information available for their spouses.
I've made minor changes to most of the sections in the previous issue to include new telephone numbers, and have included a list of web addresses for the various Alcatel-Lucent benefit groups.
Our medical plan has changed dramatically in 2008. Accordingly,
Section 5 - Health Insurance & Prescription Plan, has been completely revised. The Medical Benefits (See Section 5.1) portion (doctors, etc) has remained essentially the same, but the Prescription Drug Plan (See Section 5.2) has changed considerably.
Included in this revision is a review of Medigap Plans and Medicare Part D Prescription Plans as possible replacements for the current Lucent Medical Plan for a survivor
I hope that this information will prove of value to you and your spouse.
